Ownership change of Retail drug license in Bangladesh

List of required documents for the ownership change of pharmacy shop and model pharmacy in Bangladesh:

1. Application by the current owner & application by the new owner (NID of Both parties and 2 copies photo)
2. Inspection report of the pharmacy with recommendations
3. Copy of the drug license (attested)
4. Copy of the trade license (attested)
5. Copy of the affidavit by a notary public/1st class magistrate for the purpose of ownership transfer (attested)
6. Govt. fees & 15% VAT for ownership change-original copy
7. Certificate copy and the Agreement/Declaration of commitment of the registered Pharmacist
8. Bank solvency certificate-original
9. Tax return submission copy & TIN certificate copy
10. Deed/rent agreement/holding tax copy of the shop
11. Photo of the shop (3-4 photos)
